Certificate of Birth Resulting in Stillbirth / Certificate of Stillbirth
​Certificates can be requested after you have been discharged and your hospital/physician has filed the necessary paperwork. At your request, your State Office of Vital Records can issue you a "Certificate of Birth Resulting in Stillbirth" or "Certificate of Stillbirth" to record and memorialize the birth of your stillborn child. Because laws vary by state, so do the certificates available to you. Some states offer certificates for babies less than 20 gestational weeks old. A letter to the addresses below will stop about 75% of baby-related mail during the first year after a baby's death. The Mail Preference Service division of the Direct Marketing Association (known as DMA) compiles a list of people who do not want junk mail. DMA provides their compiled 'Do Not Mail' list to any company that requests it.

Tips: Write to Mail Preference Service and request your name and address be deleted from all mailing and marketing lists. Provide them with three spellings or variations of your name, as well as with any other names at your address (including previous occupants). They will retain your information for five years. If you wish, at that time you should write again. Be sure to write when you move. Include your name and address. Be sure to sign and date your letter.
